Sliding Fee Program
HHS provides a Sliding Fee Discount Program to reduce barriers to accessing care by offering services at a discounted fee to those that qualify. This program is available to those with and without insurance. Eligibility is based on family size and income.
To apply for the sliding fee discount program patients must:
1. Download and complete the Sliding Fee Discount Application. Paper applications are also available at all HHS locations. An application can be mailed to you, upon request.
2. Provide the required proof of income for all household members.
3. Return the completed application, along with all required documentation, to any of our centers for processing. Applications can also be mailed. Mailing addresses can be found on page 1 of the application.
Patients cannot receive the sliding fee discount for services until enrollment in the program is completed.
The Federal 340B Drug Pricing Program allows HHS to buy medications for our patients at discounted prices. We sell those medications at discounted prices to our patients at local pharmacies. This makes medications more affordable for uninsured and underinsured patients. This program is only for HHS patients and only for prescriptions written by an HHS provider. Currently, HHS provides these medications at four (4) local Walgreens locations and Back Home Pharmacy in Waverly.
In addition, HHS Care Managers will assist patients with filling out applications to submit to drug companies which have special programs to provide certain medicines at no cost to qualifying individuals.
